By visiting Florencia Skin Care location, you acknowledge and accept our terms and conditions. If you do not agree to be bound by these terms and conditions, you may not access Florencia Skin Care and it’s services.
• Release and Consent
You accept full responsibility for the service(s) rendered at Florencia, Inc. (DBA Florencia Skin Care) and release Florencia Skin Care of any/all liability for any possible outcomes, whether known or unknown, foreseen or unforeseen. You acknowledge and agree that you are responsible for your own health, and that Florencia Skin Care aestheticians and practitioners are not health care practitioners and cannot be expected to diagnose and/or treat individual health issues. You understand that you are responsible for discussing any questions that you may have concerning your health conditions (if any) during the course of your treatment. By voluntarily choosing to receive Florencia Skin Care treatments, you warrant that to the best of your knowledge, you have no condition, impairment or ailment that prevents you from receiving such treatment(s). You hereby forever release, covenant not to sue, discharge, and hold harmless Florencia Skin Care (and Florencia, Inc., subsidiaries, affiliated corporations, and their respective officers, directors, shareholders, employees, and successors) and waive any and all claims, personal injuries, liabilities or damages of any kind, and costs or expenses, including but not limited to claims of negligence of Florencia Skin Care, that you may experience directly or indirectly in connection with your attendance at Florencia Skin Care or participation in Florencia Skin Care services.
• Consent Forms:
Upon booking your appointment or your arrival at Florencia Skin Care, the following forms are required and you are responsible for providing accurate contact and health information.
– Release & Consent Form- You acknowledge that the information you provide on your Release and Consent Form is true and accurate, and you agree to all terms and conditions in your Release and Consent Form and understand that you are waiving valuable legal rights. If any changes are made to the information you have submitted on that form, you agree to verbally disclose those changes to your therapist. YOU AGREE TO AUTO-RENEW YOUR AGREEMENT WITH THE CURRENT RELEASE AND CONSENT FORM EACH TIME YOU ENTER OR USE Florencia Skin Care WITHOUT PROVIDING A SIGNATURE EACH TIME.
– Minor Form: to be filled out by a legal guardian for minors age 12-17.
• Assumption of Risk, Release and Waiver of Liability, & Indemnity Agreement Relating to Coronavirus/COVID-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. Evidence has shown that COVID-19 can cause serious and potentially life threatening illness and even death. Florencia Skin Care has put into place preventative measures to attempt to reduce the spread of COVID-19. You acknowledge that you must comply with all set procedures to reduce the spread while at the spa. You have read and understood the above warning concerning COVID-19. You understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of yourself and others, including, but not limited to, Florencia employees, Florencia guests, and Florencia vendors. You acknowledge the contagious nature of COVID-19 and you hereby voluntarily agree to assume all of the foregoing risks and accept sole responsibility that you may be exposed to or infected by COVID-19 in order to utilize Florencia services and the Florencia premises.
• Arbitration Policy
You acknowledge that the exclusive method for resolving any controversy, dispute, or claim arising out of or with respect to my dealings with Florencia Skin Care shall be by Binding Arbitration. Such arbitration shall be before and in accordance with the rules and regulations of the American Arbitration Association. In addition, you and Florencia Skin Care each agree that any disputes shall be arbitrated only on an individual basis and not as a plaintiff or a class member in any purported class, representative proceeding or as an association. The Arbitrator does not have the power to vary these provisions.
• Section 1542 of the California Civil Code
The Florencia Skin Care corporate office is based in California and Section 1542 of the California Civil Code states: “A general release does not extend to claims which the creditor or releasing party does not know or suspect to exist in his or her favor at the time of executing the release, which if known by him or her must have materially affected his or her settlement with the creditor or released party.” You hereby waive any right or benefit that you may have under this provision, or any similar provision of any other state’s law, and understand that you are giving up claims that you may not know or suspect to exist.
• Severability Clause
If a provision of this Agreement is or becomes illegal, invalid or unenforceable in any jurisdiction, that shall not affect the validity or enforceability in that jurisdiction of any other provision of this Agreement; or the validity or enforceability in other jurisdictions of that or any other provision of this Agreement.
• Policy on Misconduct
You understand that making any illicit or sexually suggestive remarks or advances during your visit will result in the immediate termination of the session and you will be held liable for the payment of the scheduled treatment. Furthermore, the therapists and employees reserve the right to refuse services to you for any reason that they deem necessary. See our Code of Conduct policy.
• Florencia Skin Care operates by appointment only.
• At Florencia Skin Care, the comfort and safety of our esteemed guests and valued employees is our utmost priority. As such, in addition to our published policies and procedures, we kindly request your participation in the following Code of Conduct guidelines:
– Completion of accurate and pertinent health information regarding your visit.
– Clear communication of preferences, expectations and concerns pertaining to your service.
– Treatment of all spa staff and fellow guests with courtesy and respect.
• Florencia Skin Care is designed and operated for adult clientele seeking a quiet, tranquil atmosphere.
Parents, please make other arrangements for your children while you are Florencia’s guest, unless the child has an appointment.
Please be considerate of other our guests and mute your cell phones.
Florencia Skin Care reserves the right to refuse or discontinue service to anyone demonstrating behavior that is perceived to be inappropriate or disruptive in a professional setting, including but not limited to the following:
– Repetitive tardiness or late cancellations outside of our policy
– Appearing or being under the influence of drugs or excessive alcohol consumption
– Engaging in sexually explicit conversations, comments, jokes or innuendos
– Displaying any form of discriminatory behavior
– Revealing overly personal information that makes the staff or other guests uncomfortable
– Requesting private information from our staff members
– Offering gifts or additional payments outside of our standard fees and gratuity process
– Engaging in financial negligence

• Cancelation Policy: Florencia requires 48 hours notice in event of a cancellation. If you miss, cancel or change your appointment outside this designated time, you will be charged 50% of the total of your scheduled services. Late arrivals may result in a shortened appointment or rescheduling (charges may apply). To cancel or change your appointment, please call the Florencia Skin Care directly at 707-789-0168.
• Our prices of services and products may change without prior notice.
• Gift Certificates are not refundable or redeemable for cash or refund. Florencia does not replace lost Gift Certificates. In order to redeem a Gift Certificate, it must be present at the time of redemption at Florencia for services or at checkout page online.
• Shipping Policy: Most orders will ship within 3-4 business days. Most in–stock items will arrive within 7 – 10 working days. Include your daytime phone number when ordering. Fragrances cannot be shipped to Alaska, Hawaii, Catalina Island, international destinations. ALL SALES ARE FINAL. Please read our Shipping & Return Policies
• Return Policies: If you need to return merchandise, please call us at 707-789-0168 or email us at info@florenciabeauty.com. Once we approve your request, we will provide you with a Return Authorization Number. You may then send us the unopened and unused merchandise along with your Return Authorization Number.
In – Store Return Policies: unopened & undamaged product returns must be accompanied with the receipt within 7 days of receiving products and will be honored with store credit. Opened products are not refundable and not returnable.
Claims and shortages must be reported within 24 hours of delivery. All packing materials must be saved. You are responsible for the shipping charges on all returned items. Returns due to customer absence or refusal are subject to 25% restocking fee.
